Our Practice is certified as a Patient Centered Medical Home. Practices certified as PCMH utilize a team based approach to coordinate care and services outside the primary care office with the primary care practice serving as the foundation.
PRIMARY CARE
Our internal medicine practice offers a team approach to primary care services. We provide comprehensive primary care with an emphasis on addressing the unique health needs of each patient. We are an ADA Education Certified facility that focuses on utilizing technology to our patient’s advantage.
SPECIALIZED DIABETES MANAGEMENT
Our practice consists of an award winning team that includes an Endocrinologist and Certified Diabetes Educators. We provide comprehensive care for patients with Diabetes. We use the latest diabetes treatments and technology to empower you to take charge of your diabetes and prevent complications.
Services include diabetes and nutrition education, retinal eye exams, continuous glucose studies, foot exams with circulation testing, and care management.
PAIN MANAGEMENT
Pain can hold you back from living your life to the fullest and keep you from accomplishing certain daily activities. Our pain management team utilizes a variety of pain relief techniques to help manage patients struggling with chronic pain. Our pain management team offers holistic treatments for patients looking for alternatives to pain medications.
CARDIOLOGY
Our on-staff cardiologist is available for our cardiology patients in finding, treating and preventing diseases of the heart and blood vessels.
Cardiologist hours are the second and fourth Fridays of each month, but times may vary. Please call 313-885-2334 for details.
TELEHEALTH BEHAVIORAL HEALTH
